Default namespace XHTML not appearing in namespace-uri-from-prefix()

Description

When the default namespace has been set to xmlns="http://www.w3.org/1999/xhtml" some (but not all) browsers return "" for namespace-uri-from-prefix('',$context) - Chrome does, Firefox doesn't - it returns the XHTML URI.

The issue appears that element.lookupNamespaceURI("") isn't consistent across browsers. https://www.w3.org/TR/DOM-Level-3-Core/core.html#Node3-lookupNamespaceURI suggests using null to recover the default namespace.

Patch applied to CoreFn.namespace-uri-for-prefix
